REGISTRATION DOCUMENTS
ARE AVAILABLE ON THE SLA WEBSITE UNDER THE DOCUMENTS SECTION IF YOU WANT TO COMPLETE THEM BEFORE YOU COME. The Medical Waiver does not need to be filled out for
registration but does need to be completed and turned in at the first practice.
